Understanding Our Personalized Wellness Plan
Do you ever feel a lack of energy, loss of physical or mental function, low libido, or fatigue? With our wellness plan, we hope to provide you with options that your normal primary care physician cannot. As a plan member, you will have access to all of our resources, educational tools, and health management services for a lifetime of comfort and convenience.
Luckily, we provide a comprehensive wellness plan program that involves the following:
- Scheduled visits with our physician: After you meet with our physician and chronic care health coach, we will schedule a one-on-one wellness visit at our office to follow-up.
- Wellness assessments: We will provide you with a series of assessments and tests, that way we can collaboratively determine a baseline of your current health status.
- Create your personalized wellness plan: From there, you and your doctor will develop a personalized wellness plan designed with your specific needs in mind.
